CKAM Community Feedback Survey

CKAM would like to hear from families! We are looking for your ideas, suggestions, and feedback on how we can better support parents, caregivers, youth, and children in our community.

Who Can Participate:

You must be a Driftpile Cree Nation member.
You must be a parent or caregiver of a child, youth, or children.
Parents may complete the questionnaire on behalf of their children. (Only one entry per person is allowed.)

Incentives:

All completed surveys will be entered into a random prize draw.
Receive a double entry if you follow us on Facebook!

Important Reminder:
Responses that contain derogatory language, profanity, or inappropriate content will be immediately discarded. Participants submitting such responses will not be eligible for the prize draw.
